How to Obtain a World Wide Medical Device
All Neurostimulator devices distributed by World Wide Medical Services are FDA regulated and require a physician’s prescription (or Letter of Medical Necessity) before being dispensed to the patient. For convenience, the necessary paperwork is packaged along with the device being sent to the medical facilities.
For The Medical Facility
As a standard required by Medicare and practiced by group health carriers, all patient files are required to have the Patient Data and Assignment of Benefits Form completed and signed by the patient or patient’s caregiver. In addition to this form a signed prescription (or Letter of Medical Necessity) can be faxed from the medical facility to facilitate the process of verifying benefits. Once received and insurance verified, the patient is able to take home the device.
For Patients Who Are Not Currently Being Treated For Pain Management
If you do not currently have an attending physician treating you for chronic or acute pain, and you wish to have an evaluation to determine if your condition meets criteria for one of our devices, print out the Patient Data & Assignment of Benefits Form and Letter of Medical Necessity and bring them with you during your visit with your physician. Once filled out, these forms can be faxed to 877-552-3422.
